Nacha's definition

The RDFI has been notified by the Receiver that the Originator and the Receiver have a relationship and an authorization to debit exists, but there is an error or defect in the payment such that the entry does not conform to the terms of authorization (for example, the entry is for an amount different than authorized; the entry was initiated for settlement earlier than authorized; the entry is part of an Incomplete Transaction; the debit entry was improperly reinitiated; the Reversing Entry was improperly initiated by the Originator or ODFI; the Receiver did not affirmatively initiate a Subsequent Entry in accordance with the terms of the Standing Authorization.

Nacha marks this ISO code as mapped to more than one Nacha return code, so the ISO code alone does not identify the Nacha reason; read the narrative or the original return record.

Nacha's table prints NARR without the shared-mapping asterisk on this row, but NARR is the mapping it gives R06, R12, R14, R17, R23, R30, R31, R34 and R36 as well, so the ISO code alone does not identify an R11.

Return time frame

Nacha's rule page states the return time frame is 60 days, and that the RDFI is required to obtain the Receiver's Written Statement of Unauthorized Debit. R11 returns count toward the unauthorized entry return rate and are covered by the Unauthorized Entry Fee.

Can the entry be reinitiated?

Nacha's rule page states that with an R11 return an Originator is permitted to correct the underlying error, if possible, and submit a new Entry without being required to obtain a new authorization — and that the new Entry must be originated within 60 days of the Settlement Date of the R11 Return Entry. The new entry carries the same ODFI warranties as any other.

Unauthorized return rate

Nacha counts this code toward an Originator's unauthorized entry return rate, whose threshold Nacha set at 0.5 percent. Each return under this code moves that ratio for your ODFI, independently of the money involved.

What an operator does next

An authorization exists but the entry did not match it — wrong amount, earlier than authorized, an improper reinitiation, part of an incomplete transaction. Correct the error and, if you can, originate a new entry within 60 days of the R11 return's Settlement Date without needing a new authorization. It still counts toward your unauthorized rate, so fix the process that produced the mismatch, not just the entry.
